We believe that dementia / Alzheimer’s does not define a human being and each person is a unique individual with a rhythm of life all their own. Building a positive relationship with each client, we put the focus on that rhythm creating an environment where each person can thrive in their own environment.
Our approach is personalized to fit the unique preferences, needs and interests of your loved one. We start by getting to know them and their needs better by conducting our Rhythms of My Life interview. Using this information, we’ll create a customized at home dementia support plan that focuses on the strengths of each individual, creating comfort, knowing personal triggers, desired lifestyle and what brings your loved one joy.
Each team member receives specialized training to assist individuals receiving our memory care services. In fact, many of our team members would say that our ongoing training and our longevity in the healthcare industry is what sets us apart. We pride ourselves in taking a holistic, hybrid approach to training our care partners. Our training model not only offers online flexible, continuing education courses but also the human element we believe is critical to high-quality care and best learned during our in-person seminars. We believe our comprehensive training and unique approach ultimately enhances the effectiveness and application of knowledge for person-centered care resulting in positive outcomes.
We ensure every team member learns that dementia is a shift in the way a person experiences the world around him/her and affects every area of their lives, not just memory. We also provide annual competency training for our PCA and CNA caregivers.
As a “whole community” disease, dementia affects not only the person living with it, but it affects the families and communities as well. We believe that families and communities have an important role to play in supporting those living with dementia and it’s just as important to educate and support them so they, in turn, can offer support to others on the dementia journey.
Our innovative Rhythms philosophy and approach to memory care is deeply rooted in our memory care principles and beliefs.
Each of these principles guides our actions embracing individual differences, identifying and accommodating your loved one’s unmet needs, reinforcing a sense of purpose and understanding and validating your loved ones feelings. Our goal is to create an environment where your loved one can navigate the world successfully, and live in safety, comfort and dignity in their later years.
Being at home in familiar surroundings can be beneficial to older adults living with dementia and Alzheimer’s in the earlier stages of cognitive and memory changes, That’s why our compassionate team members strive to do everything we can to make your loved one feel comfortable and safe as we come to you and offer care and assistance to you and your family at home. We are dedicated to supporting you and your loved one by focusing on what is most important to your and your loved one, enhancing their quality of life and personal freedom.
